adds "-G column-formats" option to tshark and fixes documentation

This patch adds the "-G column-formats" option to tshark and adds that to the
documentation.  While I was in there, I noticed that several of the existing
glossary options were missing from either the documentation, from tshark's own
"-G -?" listing or both, so those missing items (ftypes, heuristic-decodes,
plugins) were also added.
