With the bug database I referred to, it was done manually by the provider of the information.

I think it worked pretty well in practice, although people did forget on occasion. Of course, internal people who filed the bugs pretty much knew the rules, and for externally-filed bugs, Developer Technical Support (ooh what a giveaway) may have updated the status when the external developer provided the information.
