The changes are logically not too complex, but the patch size may look daunting
(mainly due to renames/structure indirection).

Without this patch, STARTTLS dissection is in a horrible state for 1.12.x (and
older), so I am tempted to propose it for backport. Though there have not been
bug reports other than this one, so it is probably fine to wait for Wireshark
2.0.
