ssl: fix ECDH server key exchange for TLS 1.2

In TLS 1.2 the signature and the hash algorithm was added to the
digitally-signed structure, see RFC 5246,  4.7. "Cryptographic
Attributes", this also affects ECDH, see RFC 5246 A.7. "Changes to RFC
4492".
