Revision 42254, revision 45389 and several others have all been reverted. The
ephemeral allocator was getting extraordinarily complicated as we tried to work
around the various bugs that kept popping up due to the multiple attempts to
fix this one.
Jakub says he has a much simpler and safer fix for this bug based on Anders'
idea in comment #26, so hopefully that will do it.
