DVB-CI MMI resource, patch against r37701

Compiled (64-bit) with GLib 2.16.6, with libpcap 0.9.8, with libz 1.2.3.3,
without POSIX capabilities, without libpcre, without SMI, without c-ares,
without ADNS, without Lua, with Python 2.5.2, with GnuTLS 2.4.2, with Gcrypt
1.4.1, with MIT Kerberos, without GeoIP.
Running on Linux 2.6.29.1, with libpcap version 0.9.8, with libz 1.2.3.3.
Built using gcc 4.3.2.

The attached patch (against trunk r37701) adds the MMI resource as defined in
the DVB-CI specification (EN50221), section 8.6 (low-level MMI is excluded as
it's not used in practice).
The patch allows dissection of all text boxes that a DVB-CI host displays on
behalf of the CI module.
I added a pcap file with a compilation of my test messages, it's not sequence
that was actually captured but it covers common protocol errors.
