Currently the Infiniband dissector incorrectly identifies management datagrams
according to their virtual lane: Only packets transmitted on VL15 are
considered to be MADs. This patch fixes the code to correctly identify MAD
packets by their source and destination queue pairs.

The code has undergone fuzz testing.

This patch is submitted on behalf of Mellanox Technologies.
