Good question. Searching for "ext" in the 08.08 spec shows me two occurences.
One in the "Cause" flag where the '1' indicates that two octets are used for
the cause and the ChanType (with the overload for speech, signalling and data).

I will prepare a patch that will use the new value for the ChanType and another
one to introduce a new flag for the Cause.
