Proposed patch to support dissecting bonded initial ranging requests.

I tested this dissector using a new DOCSIS 3.0 modem in a UEPI configuration
(Upstream External Phy Interface).  The UEPI dissector invokes the DOCSIS
sub-dissector as if it was a normal DOCSIS frame.
