The tools/make-manuf script attempts to gather Ethernet codes from IEEE, but
also from CaveBear at http://www.cavebear.com/CaveBear/Ethernet/Ethernet.txt,
but unfortunately this link is dead.

I could not find any meaningful contact information to Karl Auerbach on the site
other than Santa Cruz, CA, which isn't all that helpful.

CaveBear's, "Ethernet Codes Master Page" contains Ethernet code links to other
sites and mirrors, but the only one that seems to work is the MIT one, namely
http://www.mit.edu/~map/Ethernet/Ethernet.txt.  From what I can tell though,
that page hasn't been updated since 1999/03/09, so that's also not very helpful.

So ... should we use the MIT link instead, try to contact Karl somehow, or just
eliminate the CaveBear URL altogether and stick with IEEE?

I was about to commit a change to remove it, but I figured I'd check here first.
