ieee80211: Split add_fixed_field() into smaller functions

The app_fixed_field() function has grown to overly complex and long
function. Split it into separate helper functions for each fixed field
and a table of dissector functions. This makes it easier to extend and
maintain the implementation.

(the new version is otherwise identical to the earlier attachment 7131 but it
is rebased on top of the current trunk to make it apply again)
