I haven't dug into the code much yet, but in each case where you noted the
tracking not working, there are two ping requests in a row, such as 39-42:

39 169.203.4.3 -> .4.1 matches 42, but not tracked
40 192.168.4.3 -> .4.1 matches 41, tracked
41 192.168.4.1 -> 4.3 matches 40, tracked
42 192.168.4.1 -> 4.3 matches 39, but not tracked

Is that the reason?  That the second ping between hosts is overwriting some
variable?  So does it keep track of when multiple pings have gone betwen the
same two hosts?
