Add /MD switch to portaudio compile in gtk/makefile.nmake to fix MSVC warning

with c-ares, without ADNS, with Lua 5.1, with GnuTLS 2.3.8, with Gcrypt 1.4.1,
with MIT Kerberos, with PortAudio V19-devel (built Aug 10 2008), with AirPcap.

Running on Windows XP Service Pack 2, build 2600, with WinPcap version 4.0.2
(packet.dll version 4.0.0.1040), based on libpcap version 0.9.5, without
AirPcap.

When linking wireshark.exe, the compiler prints the following warning:

LINK : warning LNK4098: defaultlib "LIBC" conflicts with use of other libs; use
/NODEFAULTLIB:library

This appears to be because the portaudio objects are being compiled without the
/MD switch, and are being linked with LIBC.lib.  Adding the /MD switch to the
portaudio compile lines in gtk/makefile.nmake resolves this (patch attached),
and eliminates the compiler warning.
