Updated Infiniband 1.2.0 plugin

Replaces tvb accesses with accessor functions.

Better attempt to handle RWH headers and call ethertype() rather than manually
testing only a few ethertypes. An example trace is needed to test this.

Still slightly confused where tvb_length() or tvb_reported_length() should be
used. tvb_length is the actual length, e.g. caplen, whereas tvb_reported_length
is the expected length, e.g. wlen?
